1. Eligibility
1.1 Participants must be 18 years or older at the time of entry.
1.2 The survey is open to legal residents of the UK and The Republic of Ireland and EU.
1.3 Employees of TympaHealth, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members are not eligible to participate.
2. Participation
2.1 Participation in the survey is voluntary. By entering the survey, participants agree to be bound by these terms and conditions.
2.2 To enter the prize draw, participants must complete the survey in full and submit it before the closing date 11:59 PM 31st October 2024.
2.3 Only one entry per person is permitted. Multiple entries from the same person will result in disqualification.
3. Survey Prize
3.1 The prize for the survey draw is £100 Amazon Voucher.
3.2 Prizes are non-transferable, non-refundable, and no cash alternatives are available.
3.3 TympaHealth reserves the right to substitute the prize with an alternative of equal or greater value if necessary.
4. Winner Selection and Notification
4.1 The winner will be selected randomly from all eligible entries within 3 working days after the survey closing date.
4.2 The winner will be notified via email or telephone within 7 days of the draw.
4.3 If a winner cannot be contacted or does not respond within 7 days of notification, TympaHealth reserves the right to withdraw the prize and select an alternative winner.
5. Data Collection and Privacy
5.1 By participating in the survey, participants consent to the collection and processing of their personal data by TympaHealth for the purposes of administering the survey and prize draw.
5.2 TympaHealth will handle all personal data in accordance with its privacy policy https://tympahealth.com/privacy-notice/
5.3 Personal data will not be shared with third parties without the participant’s consent, except where required by law.
5.4 Participants may be contacted by TympaHealth for follow-up purposes related to their survey responses.
6. Disqualification
6.1 TympaHealth reserves the right to disqualify any participant who violates these terms and conditions, provides false information, or engages in fraudulent activity.
6.2 Entries that are incomplete, illegible, or corrupted will not be accepted.
7. Liability
7.1 TympaHealth is not responsible for any technical issues, malfunctions, or other problems that may result in lost, incomplete, or misdirected entries.
7.2 TympaHealth accepts no liability for any damage, loss, or injury suffered by participants as a result of entering the survey or accepting the prize, except where such liability cannot be excluded by law.
8. General Conditions
8.1 TympaHealth reserves the right to amend or cancel this survey and prize draw at any time without prior notice.
8.2 In the event of any dispute regarding the survey or prize draw, TympaHealth’s decision shall be final and binding.
9. Governing Law
9.1 These terms and conditions are governed by the laws of the United Kingdom and any disputes will be subject to the exclusive jurisdiction of the courts of the United Kingdom.
